The hip is a ball and socket joint. The ball is the head of the femur and the socket is the acetabulum of the pelvis. Both the acetabulum as well as the head of the femur are covered with a layer of hyaline cartilage. The hip is constructed such that multiple ligaments and muscles provide stability for the joint.

Unfortunately, the peculiar angulation of the hip contributes to steady worsening of osteoarthritis once abnormal forces come into play and cartilage begins to wear.

The progression of osteoarthritis often leads to the formation of bony spurs called osteophytes. These osteophytes may eventually cause what is termed "femoroacetabular impingement" or FAI.

These osteophytes develop in two distinct locations: either at the outside rim of the acetabulum or at the junction between the ball and neck of the femur. When spurs develop on the acetabulum and cause pinching with hip movement this is called a "pincer" deformity. When the spurs on the femoral head/ neck cause pinching, this is called a "CAM" deformity.

Symptoms of osteoarthritis of the hip include pain in the groin and/or buttock that is aggravated by internal rotation of the hip such as occurs with walking. Pain may also radiate down the front of the thigh to the knee. As pain progresses, nighttime discomfort becomes an issue. Shortly thereafter, restriction of range of motion develops.

Treatment, to date, has been primarily symptomatic including analgesic and non-steroidal-anti-inflammatory drugs, glucocorticoid injections, massage, chiropractic, and physical therapy.

Surgical remedies have included osteotomy (where a wedge of bone is removed to line the joint up better), resurfacing (a modified replacement), and total joint replacement. While surgical treatments are by and large effective, they are irreversible and they do carry attendant risks, some life-threatening.

So the search has been to develop treatments that will be effective but also preserve the joint. One such approach is the use of autologous stem cells. In our hands, the procedure which involves the use of autologous stem cells (a patient's own stem cells) along with autologous fat, and growth factors has been relatively successful. [At the same time, osteophytes are trimmed using a special fenestration technique.]

Quite frankly, so far, our hip results have not been as good as with the knee. There may be a number of factors involved including patient selection, the difference in joint mechanics, the fact that the iliopsoas bursa- a large cushioned sack- sits in front of the joint and stem cells may dump out into the bursa rather than staying in the joint, inability to go at complete non weight-bearing after the procedure, and so on.

We are continuing to modify our approach to this technique and our patient results are now beginning to approach what we initially hoped.

Nonetheless, the procedure needs to undergo constant improvement to eventually achieve the results we want and the patient deserves.

Doctor...My Body Hurts. There's Pain In Every Joint... What's Causing It?


Joint pain causes can be divided into several categories.

Infections such as viruses (in particular, parvovirus) can cause a type of arthritis. Bacteria such as gonococcus (the bug that causes gonorrhea) and Borrelia, the organism that causes Lyme disease, are well known infectious causes for joint pain.

Trauma is an obvious cause. This can range from an athletic injury occurring more than 30 years ago leading to osteoarthritis in the knee to a whiplash injury that happened yesterday accounting for total body stiffness and pain today!

Disorders of the blood are an uncommon but important cause of joint pain. Leukemia and lymphoma can present as aches and pains. Sickle-cell anemia and hemophilia both are associated with arthritis. The No Pain, No Gain Principle - Fact Or Fiction?


We've all heard this expression. But is it true?

This motto was first aired in 1982, when actress Jane Fonda produced a series of workout videos (you know the ones!), in which she used phrases like "No pain, no gain," and "Feel the burn" to encourage her fat-burning viewers to work harder during exercise. And it's still used today. Thousands of personal trainers repeat this mantra to their clients daily, reminding them that there is no meaningful reward without sacrifice.

There's only one problem. Pushing yourself through pain can be dangerous. Just ask Tiger Woods. Rather than take some time off & rehab the cartilage injury in his knee, he decided to keep on playing "through the pain". You can see him clutching his knee in agony after this unwise decision.

Here's another example. A young woman came in to our office with pain in her right hip. She had been training to run a marathon, and her speed and endurance were improving nicely. Out of the blue, her hip started hurting when she ran -- even during the first few steps. The pain progressively worsened, eventually preventing her from running at all.

Her doctor had told her that she had a tendinitis in her hip, and said she should be fine after a little stretching, rest & ice. That didn't seem to help.

After performing an exam & x-ray of her hip, we suspected a stress fracture. An MRI confirmed this. Less than two months of rehab later, we had her running again, and her hip was healthy.

Here's the take-home point. If this young woman had followed the "No pain, no gain" theory and continued to run, she could have developed avascular necrosis of the hip, which could lead to a catastrophic fracture of the hip, and then required her to have a total hip replacement surgery. Not a very pleasant experience.

In fact, joint pain is never normal. Pain is like the red light on your dashboard, telling you when something is going wrong & needs attention. If your engine was overheating, would you keep on driving? Would stomping on the accelerator solve the problem?

We have a simple pain rule for our patients, called the "Rule of 3". Following this rule can help you avoid injury during exercise or everyday life. Here's how it works:

The Rule of 3

During any activity, rate your pain on a scale of 0 - 10, where "0" is no pain, and "10" is the worst pain you've ever experienced in your life. As long as you keep your pain rating at 3/10 or lower, you're almost always safe. If your pain reaches levels of 4/10 or higher, either modify the activity to bring it down to a "3" or lower, or simply stop the activity. (NOTE: This rule does NOT apply to "exercise discomfort" -- the burning feeling in muscles from straining during exercise.)

And, if you find yourself having to modify or eliminate activities on a regular basis, it's time for you to meet with a qualified health professional. Pain that occurs routinely, or pain that becomes more frequent or severe, is a sign that some underlying problem needs to be corrected. And with many conditions, the sooner you get the proper treatment, the easier it is to correct.

So, let's change the old saying, "No pain, no gain" to a new one. It's safer, it's more comfortable, and it's an easier motto to live by: "No pain, no pain!". Listen to your body -- it knows what it needs!

TMJ Cure - Can You Cure TMJ?


Does a TMJ cure exist? Some say yes, some say no. As a TMJ sufferer myself, I would say maybe. I'll talk about some cures in this article, and what I've been doing to alleviate the TMJ pain.

There are several ways you're supposed to be able to cure TMJ. One is surgery. This is a surgery where they rinse out the joint to stop it from becoming inflamed. Another surgery is where they break your jaw in pieces and reconstruct it. That doesn't sound like fun to me!

Another TMJ cure is the ever popular mouth guards, appliances or splints. While they do provide some relief to a TMJ sufferer, it doesn't "cure" your TMJ. It only provides temporary pain relief. These are the splints that hold your mouth open while you sleep. I have used these, and I stopped, because it didn't help me. It actually made my pain worse. I also drooled like a dog as I slept too.

Some folks say diet can cure TMJ. While I sort of agree with them, I don't agree that it will cure it. Too many foods like jerky or too much chewy meat can make TMJ worse. Be eliminating some of these foods, you can get some relief, but again it won't cure TMJ.

I've recently been using some exercises in front of my mirror at home. These have worked better than anything for me. I've gotten pain relief and my TMJ flare ups don't happen for more than a day, when they used to last for a week at a time. I hope as I use these I'll have the TMJ cure. The important thing to remember is that exercises like this are fixing the cause of your problems be strengthening your jaw muscles and providing a long term solution, not a band-aid to a problem.
