How Osteochondrosis (OC) Occurs in the Horse


Osteochondrosis is a joint disease that can affect all horses, but is found to be most common in Warmblood horses. It is a disease that can cause lameness and create long term problems if left untreated, but it is also expensive to get the necessary treatment. OC occurs when there is a disturbance of the physiologic process of endochondral ossification, leading to locally thickened cartilage plugs. At the same time cartilage canals disappear during chondrification. This means that the articular cartilage depends exclusively on the synovial fluid for diffusion of nutrients. Therefore, excessive thickening of cartilage can lead to certain areas lacking nutrition and hence to focal necrosis and weakening of the tissue.

Osteochondritis dissecans (OCD), as well as subchondral bone cysts, are commonly considered to be a manifestation of osteochondrosis. The epiphyseal ossification centre advances out until ossification is completed, leaving a layer of cartilage. This layer of cartilage becomes the articular cartilage. If there is a disturbance in endochondral ossification, an area of retained cartilage can be formed with a consequent defect in the bone. Cracking can then proceed in this retained cartilage to give a flap or fragment of cartilage that may contain bone. These flaps and fragments on the surface of the joint result in osteochondritis dissecans. Also, because of joint incongruity, this may lead to degenerative joint disease (DJD). Unlike bone, articular cartilage shows very limited capability of remodelling and repair after maturation. This indicates that the cartilage layer needs to develop fully and correctly during this phase. Osteochondrotic lesions may disappear due to remodelling in the juvenile horse, but lesions that are still present when the cartilage is fully matured will not repair and become a serious factor form the animal welfare point of view.

Physitis of the joint also occurs and can be described as a clinical symptom especially if swelling of the growth plates is associated with lameness; radiographs are taken to ensure that there is not a significant problem within the growth plate. Clinical signs may be divided broadly into two categories; those seen in foals <6 mo old and those seen in older animals. Often the first sign noted in foals is a tendency to spend more time lying down. This is accompanied frequently by joint swelling, stiffness, and difficulty keeping up with other animals in the paddock. An accompanying sign may be the development of upright conformation of the limbs, presumably as a result of rapid growth. Fetlock osteochondrosis is particularly seen in younger foals (<6 mo old). Lameness in older horses is not always apparent, but more than likely a horse will show signs of stiffness, reduced flexion and even muscular atrophy where movement had be compromised.

Dogs suffering from dysplasia are not as active as they could be due to significant pain. Often, the only cure for dysplasia is costly surgery to replace the dysplastic hip joint.

Labs also suffer from the risk of knee problems. A "luxating patella" is a common occurrence in the knee that results in the leg being "bow "shaped. This abnormality can cause abnormal wear and tear on the joint, resulting in the development of painful osteoarthritis for some Labs.

The only way to decrease the prevalence of these disorders is through conscientious breeding practices. However, even the most vigilant breeders cannot eliminate these genetic disorders altogether. There's no way of knowing whether an animal will develop a joint disorder later in life.

Can Bodyweight Exercises Fix Creaky Joints?


Recently, I asked a few of my past customers why they decided to start an exercise program consisting of bodyweight-only exercises.

Most of the answers I received contained the expected answers.

A person wanted to get in shape, build muscle, lose fat. Some people hate going to the gym. Others simply don't have time.

But one person told me that the reason he decided to start a bodyweight only exercise program is because his joints were hurting from weight lifting and he heard that bodyweight only exercises can help to heal joints.

That Got Me Scratching My Head...

At first, I thought the idea of body weight exercises "curing" joint pain sounded like wishful thinking.

But then I remembered that my own joints and tendons actually seemed healthier and stronger when I was doing these body weight only exercises.

Was there actually something to this?

I decided to find out.

Weight Training & Joint Pain:

Partners In Crime

First of all, let me say that I like weight training and I am not in any way saying that weight training is bad for you.

However, as any long time weight trainer will tell you... your joints start to take a beating after extended periods of weight training.

Especially if you are training with heavy weight and low reps.

Usually, this type of joint pain is not permanent and will subside once the trainer takes a short break from weight training.

How Bodyweight-Only Exercises Can Help

There is nothing magical about bodyweight exercises that will suddenly make all your aches and pains up and vanish.

However, there are a few characteristics about bodyweight exercises that can dramatically improve joint health and offer relief from the aches and pains associated with weight training.

The first characteristic of bodyweight exercises is a lighter load.

If you go from benching 400lbs and instead start doing push-ups using just your own body weight as resistance, your joints will experience a tremendous amount of relief as they are no longer subject to the heavy load of weight training.

How Acid Can Heal The Burn

The main reason that bodyweight exercises seem to help alleviate joint pain is because they are usually done with higher reps (more than 15 per set).

High rep ranges cause the body to create lactic acid. Lactic acid can cause a slight burning sensation in your muscles. For example, if you run up 100 flights of stairs, the muscles in your legs will start to burn.

This burning can be slightly uncomfortable (although experienced trainers will typically enjoy the burn to some degree)... however it serves a very important purpose.

The presence of lactic acid actually stimulates the body to repair tendons and can therefore lead to increased joint health.

For this reason, many experienced weight lifters may enjoy a bodyweight-only workout program while taking time out from the grind of heavy lifting.

A Bodyweight-Only Workout Program

Again, I'm not claiming that bodyweight-only exercises are any type of cure or magic pill. But many trainers use bodyweight-only exercises to relieve joint pain and prepare their body for the next bout of heavy iron work.

What is Osteoarthritis?


Osteoarthritis is the most common form of arthritis and is also known as degenerative joint disease. In this form of arthritis, the cartilage that covers the ends of the bones that connect at the joint wears away, exposing the bone and causing pain. This condition may also involve a decrease in the amount of joint fluid, called synovial fluid, that cushions the joint, as well as changes in the ligaments and muscles that stabilize the joint.

Osteoarthritis, or OA, can be a condition that develops without any pre-existing factors, or can occur as a result of another condition. When OA occurs without another condition present, it is usually associated with age, although not every older person develops OA. There is a strong hereditary component to OA, as members of the same family often develop the condition. OA also develops as the result of other conditions, such as prior injuries, congenital hip deformities, diabetes, obesity, and infections.

The primary symptom of OA is pain. Swelling is usually mild. Stiffness does occur, especially as the disease advances. A joint may become painful and stiff to the point that it becomes immobile if no treatment is provided.

OA is not curable and the damage caused is permanent. The symptoms can be controlled through conservative treatment of the symptoms, including:

*getting enough rest
*regular exercise
*diet and weight control
*joint protection techniques
*anti-inflammatory pain medication
*topical pain treatments

These techniques are often enough to slow the progression of OA and preserve the joint for many years. Chiropractic or Physical Therapy may also be helpful periodically to manage periods of increased pain or instability.

When OA becomes advanced or a joint becomes too unstable to use, surgical intervention may be necessary. One form of surgery used is a joint fusion, where the joint involved is fused together to provide stability and relieve pain. The joint becomes immobile. This type of surgery is often done on the neck or back. The other type of surgery used is a joint replacement, where the worn out ends of the bone are removed and replaced with metal ends, providing a metal joint. A hip replacement is a common example of this type of surgery.

Legg-Calve-Perthes Disease - Ayurvedic Herbal Treatment


Legg-Calve-Perthes disease is a condition occurring mostly in first-born male children in the age group of 4 to 8 years. Back Pain Caused By Track And Field


As spring arrives, many people are returning to their favorite warm weather sports. For many students and adult athletes, this means a return to track and field. It is important to educate yourself about the common injuries associated with your sport in order to guard yourself against back pain and other conditions that can take you out of competition.

A number of injuries can occur during any sport. The following describes a few common injuries associated with the various activities of track and field.

Runners

Most people, whether athletes or office workers, tend to have weaker hamstrings than quadriceps. This imbalance causes the pelvis to be pulled down in front and increases the lumbar arch. As the back arches in more, the discs in between vertebrae are compressed unevenly and the spinal joints are strained. Combined with the jolting force your body experiences as your feet pound against the ground when running, joint and disc problems become likely. Weak hamstrings are also susceptible to strains. Runners should take special care to ensure their quads and hamstrings are balanced in strength and flexibility.

Another muscle group runners should be aware of is the hip flexor group, particularly the psoas. This muscle is used when bringing the thigh and upper body closer together. The psoas connects the lumbar spine to the thigh bone and is used heavily when running. Tight, inflexible psoas muscles pull down on the pelvis and create the exaggerated lumbar arch that tight quads create. Running without regaining flexibility of the psoas will likely worsen its rigidity and exacerbate pelvic misalignment, which in turn causes lower back pain.

Regular stretching and myofascial release can help achieve muscles that are balanced in strength and flexibility. The assistance of a physical therapist is beneficial to people with sports injuries.

Hurdlers

Muscle strains are common among hurdlers, especially in the groin and thigh. Groin strain is generally a sign of either bad form or overuse. If you feel groin pain on the side of your trail leg, you may be swinging your leg out too far or too late. If the pain is on the side of your lead leg, you may be doing too many drills with the hurdles close together.

Hamstring strains are a common overuse injury acquired by hurdlers. The hamstring of the lead leg undergoes an eccentric contraction during a hurdle jump, which means it contracts as it is being lengthened by the leg's straightening. This type of contraction comes with a high risk of muscle tearing. The trail leg works hard to push the body off the ground. Hamstring strain is a sign to stop, rest, and warm up sufficiently before practicing.

More troubling than a simple muscle strain, hurdlers have to watch out for sacroiliac (SI) joint dysfunction. The sacroiliac joints form where the hip bones meet the sacrum at the base of the spine.
Injuries to the SI joints generally occur when the muscles and ligaments that support them are weakened, torn or tightened. The tremendous forces sustained by the pelvis when pushing off the ground can, over time, destabilize the pelvis, causing significant joint pain. Proper form, adequate rest time, good conditioning and sufficient warmup time can help prevent this condition.

Throwers

Whether it's a javelin, discus or shot put, throwers are susceptible to similar injuries. Throwers in track and field use muscles throughout the whole body to build momentum; javelin throwers run, while shot putters and discus throwers spin.

The most common throwing injury occurs to the shoulder. Rotator cuff injuries affect the set of muscles and tendons that surround the shoulder, giving it stability and allowing up-and-down and rotational movement of the arm. Repeated throwing can cause tears to the muscles or tendons.

Hip and lower back strains may also occur in throwers, particularly discus throwers and shot putters who twist their bodies as part of their throw. Javelin throwers may also strain muscles throughout the back when lunging forward to throw the javelin.

The best defense against throwing injuries is to avoid overworking your arm and back. Know your limits and take adequate rest periods between practices.

Many sports injuries can be prevented with warmup, conditioning, self-myofascial release and the willingness to give your body the time it needs to recover. Whatever your track and field activity, approach it in a way that will allow you to enjoy your sport for many years to come.

-Surgical Treatment: If non-surgical option fails to relief the arthritis hand pains, then surgery should be preferred. Usually surgery is the best option in providing long-term pain relief. Joint replacement is one of the popular methods in the surgical treatment.

Though these are the best three ways to cure arthritis hand pain, these can have possible site effects. Possible side effects of injections are lightening of the skin, weakening of the tendons and ligaments. Infections may be one of the major side effects of these practices.

Dog Joint Pain Caused by Arthritis


Signs of pain in a canine are not always pronounced, but there are signs that will help you discover that the dog is suffering from dog joint pain, if you know what those signs are.

Certain breeds of dogs are more prone to having joint problems as they get older, and other dogs may experience joint discomfort due to an injury or arthritis.

Joint pain may not be noticed when it first starts, but after awhile, the owner notices that the dog is acting differently. At first, the dog will have a slow day, and then will be more active the following day, and then maybe a week later, slows again; the owner will once again see the dog is acting differently, this time raising a caution flag.

If a dog always lies on the couch and this is the dog's favorite spot, one may observe the dog approach the couch, and then in slow motion, get onto the couch. The dog might even try a scoot method to avoid lifting the hind legs.

They will use the front legs as leverage, literally pulling the hind legs onto the couch to avoid pain. Another sign is the hunched look. When the dog walks, it may look like the hind legs are closer together, taking baby steps, and the rear part of the dog is almost into a hunched position.

To avoid the joint pain, a smaller step will replace a full flex of the hind leg. Observe the dog when they try to make a circle or turn, one may notice that they almost hop to complete the circle, or instead of a tight turn, they will make the turn around by walking in a larger circle.

This action is from hip pain in the rear joints that the dog is avoiding by walking or turning, and meanwhile trying to keep the weight off of the hind quarters. An owner may even think that the dog is constipated, because of the stiff hind quarters and lack of movement in that area.

The best way to determine what is causing the pain is to have a veterinarian do an x-ray of the joints. It can be from the hip bone being a flatter joint bone, as opposed to having a curve to keep the joint in place. This would then allow the joint to freely move within that area.

The best way to give a good example is: cup your left hand, then make a fist with the right hand, now place your fist into the cupped hand. That resembles a joint. Now flatten or open the left hand while keeping the fist, see how with the extra room the joint floats and does not stay in one place.

There are several different causes of why the family pet is experiencing dog joint pain. Have a vet examine the dog, and do an x-ray to determine the exact cause of pain; nobody enjoys pain, including the dog.

Gout Pain - Is There One Miracle Cure for All?


As any gout sufferer will agree, gout pain is intense and agonising. Gout can come on very quickly and with little or no warning.

The body produces uric acid from the breakdown of food. A manageable level is maintained with excess being dissolved into the blood stream, filtered via the kidneys and passed via urine. If our bodies start to produce a higher level of uric acid, a build up occurs generally in the joints. the build up of uric acid leads to the formation of uric acid crystals causing inflammation and redness.

Once an attack has taken hold, gout pain can be debilitating. The pain can be so intense that the insignificant weight of a bedsheet over an effected joint can be unbearable.

For years I was using prescribed drugs that only seemed to work sometimes. I was prescribed drugs to prevent gout and had to take it daily. Although being diligent I still suffered regular attacks. The doctor then prescribed anti-inflammatory drugs to reduce the pain!!

It seemed that I was going round in circles and getting nowhere. The preventative medication was not working and the anti-infammatory drugs were causing kidney damage!!

Chiropractic Manipulation Helps Osteoarthritis of the Hip


Osteoarthritis of the hip is a common joint disorder, which can cause pain, inflammation, and stiffness. Age is a top contributing factor for the disorder. Other factors include family history, obesity, overuse, or a previous fracture or injury. Hip pain caused by osteoarthritis is often combined with stiffness, swelling, limited range of motion, and difficulty walking or sitting for extended periods of time.

Positive lifestyle changes can help reduce the symptoms associated with osteoarthritis. Maintaining a healthy diet and weight, getting ample rest, getting your alignment checked and exercising regularly can help limit your pain and stiffness. Exercise helps with strength and movement of the joints. Swimming is particularly beneficial since it can help improve strength and mobility of the joints but is non-weight bearing.

Chiropractic care for osteoarthritis of the hip has been studied and has been recommended as an effective treatment. In a major study, "Comparison of manual therapy and exercise therapy in osteoarthritis of the hip: a randomized clinical trial," researchers compared the effectiveness of chiropractic mobilizations and manipulations of the hip joint with an exercise only program (no manipulations or mobilizations).1 The study followed more than one hundred patients with osteoarthritis of the hip in a single-blind, randomized clinical trial. Half of the participants received the chiropractic manual therapy and the other half received the exercise program. Success rates for the manual therapy group were 81% compared to only 50% for the exercise group. "The manual therapy group had significantly better outcomes on pain, stiffness, hip function, and range of motion."1

The research showed that while exercise can be beneficial for patients with osteoarthritis of the hip, that manual therapy provided by a chiropractor is superior to exercise alone and can greatly improve a patient's quality of life.

Chiropractic is a natural form of healing that uses manual therapies (commonly adjustments) and other physiotherapy modalities. A chiropractic adjustment is one of the most common forms of manual therapy used by chiropractors. In this study, adjustments were made to the hip and spine (manipulation or mobilization). A manipulation is a high velocity action performed with specific, directed force, which helps to move the joint beyond the patient's passive (or resting) range of motion to achieve its full range of movement. Mobilization is a low velocity action applied within the patient's normal (active) range of motion.

Chiropractic adjustments are intended to restore proper body alignment, range, and quality of motion. The patients in this study who received the adjustments experienced a better outcome in managing their hip pain. Further, more than six months after the adjustments, the patients were still experiencing the positive benefits of the manual therapy, suggesting the long- term effects of the therapy.

Chiropractic care is a safe and cost effective alternative to surgery. While some patients with osteoarthritis of the hip will inevitably need surgery, it is well worth the effort to a visit to a chiropractor first. Hip adjustments by a chiropractor may help you avoid or at least delay the need for surgery.

Golden Retriever Health - Useful Information on the Most Common Health Problems


As much as Golden Retrievers can bring incredible levels of joy and pride to a family, this venerable breed also comes with a few important peculiarities, most of them, sadly, pertaining to health issues. Though, Golden Retrievers have a life expectancy that can reach up to 15 years, recent studies have shown that the dogs typically die from health complications without even reaching the prime age of 10. Much like other breeds, the Golden Retriever can become subject to multiple health issues which affect not only its general health, but also its disposition and behavior.

Although not particularly a leading cause of death, a common condition afflicting Golden Retriever health is canine hip-dysplasia (CHD). This type of genetic joint disease results in the misalignment of the hip socket, a condition which in human terms can be most closely associated to arthritis. Goldens, like many other dog breeds, are quite susceptible to this degenerative disorder, causing much pain to a dog. In case the condition is more severe, hip dysplasia can result to partial lameness. As the Golden is considered one of the most active breeds of dogs, loss of motor functions is especially dangerous to its mental condition.

Less common but acutely important to Golden Retriever health are cataracts which clouds the eye and can ultimately lead to loss of vision. Largely hereditary, cataracts among canines are usually treatable, but obviously very troublesome and costly. Even an attempt to surgically treat a cataract condition likewise requires the dog in question to be in suitable health, not to mention the pain of searching for a veterinary clinic with the adequate equipment and tools necessary for the procedure. Retrievers with untreated eye problems can gradually lose the ability to see in night and low-light conditions, significantly affecting balance and temperament.

Allergies are also a constant bother to Golden Retriever health, manifesting mostly in the skin but sometimes in other body parts as well. While all dog breeds suffer from allergies, its affliction to a dog is especially noticeable as it affects the famous fur coat the breed is known for. Canine allergies typically follow flea bites although it is possible that a Golden develops an allergic reaction to food or even household items. Tests to determine allergies do exist with the most common being intradermal skin testing.

A chief and grave concern among owners is a bleeding disorder called the von Willebrand disease (vWD). Also a common occurrence among humans, the von Willebrand disease results in the inability of platelets to function properly for the process of blood clotting. The presence of this disease in a Golden typically renders the owner to be extra careful of the dog, with a typical side-effect being the avoidance of major physical activities which Golden Retrievers live for. Likewise, vWD normally inhibits the ability of a dog to undergo any kind of surgery, giving owners little to no option for treating other health conditions such as cataracts.

As major Golden Retriever health issues are hereditary in nature, new owners should take particular note of the reputation and track record of the breeders from which they plan to get a puppy from. Top-quality breeders do the legwork for diagnosing early symptoms of major diseases and health defects and will usually provide advice for treatment and consultation. If possible, asking for a health certification before taking a pup home is a very prudent option.

Unfortunately Golden Retrievers health problems are not limited to hereditary issues. Cancer is the leading cause of untimely death during last decades. Despite the fact that in some cases it can be treatable the death rate among diseased remains very high.

Otherwise, the lifespan of a Golden Retriever is highly dependent on diet options and its access to physical activities. Routine check-ups with a veterinarian are likewise advisable.

Migraine Headache and TMJ Syndrome


Headache and TMJ syndrome may go together if you have chronic recurring headaches. While wildly over diagnosed in the 1970's recently newer imaging techniques, such as MRI's, have allowed for a better look at this facial joint that causes so much pain.

TMJ or temporomandibular joint syndrome as the disorder is commonly called, is poorly understood at the present time in terms of how it is caused. While many patients have bruxism or grinding of the teeth at night, others may have had trauma, osteoarthritis, or rheumatoid arthritis. Many times the cause is unknown. For those of you with migraine, the constant clenching of the jaw causes muscle spasms in the temples. This then leads to a migraine.

The symptoms of TMJ may be any or all of the following: pain or tenderness in jaw and/or around the ear , difficulty chewing, a clicking sound when jaw is opened,spasms causing the jaw to "lock" when open or closing, uncomfortable or uneven bite, and finally, headache!

Diagnosing TMJ involves a good history of how you sleep, grinding habits during the day and general tension. An exam of the jaw should involve palpation of the joint both inside and outside of the mouth. Imaging of your jaw may be needed to find out how much damage is in the joint. X-rays, a CT of the jaw or even an MRI would assist the doctor. Once diagnosed you should be referred to a dentist who specializes in TMJ syndrome.

Treatment of TMJ may greatly reduce your headaches. But like anything else, a multi-factorial approach to the problem will yield the best results for you. Each of the following treatments may help you to reduce your pain.

Overuse of jaw muscles. This is mostly a lifestyle change. You must be willing to recognize when you are grinding your teeth during the day and learn to relax the jaw. Also, try not to eat foods that are difficult to chew until you heal a bit more.

Appliances. Depending on your particular situation, the dentist may make a bite plate or splint to help align your teeth at night. A night guard appliance could also be used to help prevent grinding.

Medications. Various medications may ease your pain and thereby reduce your headaches. A short course of steroids will reduce inflammation, or if the condition is not severe an anti-inflammatory could be used. Spasming of jaw muscles could contribute to the problem, and the ideal medication for this is Elavil.

Dental Repairs. Your dentist may recommend dental work that may not make much sense to you at first. However, correcting bite, repairing teeth and filing down tooth edges that aren't meeting correctly will all serve to decrease TMJ. Surgery.This is a final option and is reserved for cases where the above options have failed. In this case the joint is so badly damaged that joint reconstruction or replacement may be recommended. And again your headache and TMJ may be reduced!

Teeth grinding is a habit that can turn into a disorder when not taken care of properly. One of these complications is the temporomandibular joint or TMJ problems. It is a disorder that is involved with the inflammation of the temporomandibular joint. This joint is responsible for connecting the mandible to the skull, and is jointly responsible for the opening and closing of the jaw. Without it, speaking, eating and other activities associated with mouth movement are not possible. That is why problems with this joint can cause serious disabilities to an individual.

TMJ problems can be caused by a number of reasons. As stated above, teeth grinding can lead to temporomandibular joint disorder or TMD. In fact, TMD is one of the most serious consequences of teeth grinding. This is because teeth grinding can damage the muscles and joints surrounding the teeth and the jaw. An individual is said to be developing TMD from teeth grinding if he wakes up in the morning with an aching ear or jaw. Any damage or trauma to the jaw can also lead to the disorder. Like other bones in the body, when this joint gets damaged, it will be inflamed. Damaged bones take at least 2-4 months to heal. When the joint does not heal correctly, it can deform the temporomandibular joint, causing TMD. Activities that distort the jaw and the upper body can also lead to the condition. An example of this is frequent usage of telephones and awkward sleeping positions.

When TMD is suspected, the individual can exhibit a number of symptoms. The most common symptom of TMD is headache, accounting to 80% of the patients. This headache is accompanied by facial pain, aggravated by movement of the jaw. This can be explained by the damage sustained by the joint. When the joint is damaged, the trigeminal nerve can be compromised as well. This nerve is responsible for the movement of the jaws and the ability of the face to feel touch. Ear pain is also common to affected individuals.

The temporomandibular joint is located right in front of the ear, so pain in this area is very likely. Clicking sounds can also be heard when the mouth is being opened and closed, indicating misalignment of the joint. A grating sensation can also be felt if the temporomandibular joint is felt while moving the mouth. To confirm the diagnosis, the individual will undergo an X-ray to identify any abnormalities in the joint. The physician will also conduct a physical assessment of the teeth and jaws to gauge the extent of the disorder.

When a diagnosis has been made, the individual with a TMJ disorder will undergo treatment. Treatment modalities include physical therapy, medications to relieve pain, correction of the alignment of the joint and stress management. All of these interventions are geared toward the relief of the symptoms of TMD, correction of the factors that caused the disease and treatment of other factors that aggravate the disorder. Treatment can be long and difficult, so a lot of patience is needed from the individual.

Hip Joint Replacement - Expectation and Limitations


Hip Joint Replacement is a surgery commonly performed in Orthopedic practice, wherein the hip joint is changed to a new modular bearing providing painless articulation.

The common indications of a Hip Replacement surgery are:
1. Hip Joint Arthritis- either degenerative or inflammatory pathology.
2. Fracture Neck Femur (upper end of thigh bone)
3. Malformed hip Joint- a disease of the childhood
4. Loss of blood supply (Avascular Necrosis)

The definite indication of Hip Replacement is - Painful hip joint with radiological evidence of obliteration of joint space. Only exclusion is Fracture Neck Femur; wherein a partial change of the bearing component may be performed.

Patients usually have a number of queries regarding;
a) Will the pain go?
b) How much time do I need to stay in Hospital; and how much time off work?
c) Do I need to come back again for a possible surgery?
d) What happens to the metal inside my body?
e) What precautions I need to take, and for how long?

I will try to offer explanations to every question one by one...

The foremost concern is PAIN.
After a hip replacement, the pain due to arthritis usually subsides in 4-6 weeks. Thereafter, there may be occasional reminders of pain after unaccustomed activity. If however, the pain re-appears after a prolonged pain free period; or the pain tends to worsen progressively, there is a definite cause for concern. This requires an urgent evaluation by the Orthopedic surgeon.

As regards hospital stay, the usual duration is 1 week. The timing of going back to previous level of activity depends upon the type of Hip replacement - Cemented/ Un-cemented; and the quality of bone stock. Usually patients may resume office and sedentary work after 4-6 weeks.

The typical longevity of a Hip replacement depends upon- quality of bone/ level of physical activity/ Age/ body weight/ technique of surgery. Most hip replacements survive an average of 15-20 years. However, a large number of variables affecting longevity prevents accurate survival analysis.

The metal typically used in a Hip Replacement is non - reacting to body fluids. It may be Stainless Steel/ Titanium alloy, Cobalt - chrome alloy. The bearing surface is important factor in survival. Ceramic-on Ceramic and Metal-on Metal the the favored ones in current practice.

The most important of all, are the precautions to be taken by the patient himself. Dislocation of the new joint is a potential risk. extremes of movements should be avoided under all circumstances.

To summarize, it is important to understand the advantages/ limitations of a Hip replacement and to expect a reasonable goal before undergoing surgery. In a properly selected patient, it offers significant improvement in the quality of life and activity level.

Hip Pain - Do You Really Have Arthritis in Your Hips Or is it Muscle Pain?


"Oh, you have arthritis in your hips."

That's probably the most common diagnosis doctors give to us when we complain of hip pain.

If a doctor looked at an x-ray of any one of us, he would see changes in the bones. He would say we have arthritis. He would tell us that is the cause of our pain. Well, maybe it is and maybe it isn't.

We all have changes in the bones around our joints. Do we all have pain? The answer to that is no, we don't.

"You will just have to learn to live with it."

Maybe. Maybe not.

Doctors are great people and wonderful life savers lots of times, but as a rule, they don't know much about muscles. That's not something that is taught much in medical school. (Not in the schools you and I went to, either.)

We have large, powerful muscles that pass over our hip joints. These muscles allow us to move our legs and our body. The hip joint is where our thigh bone connects to our pelvic bone.

Those powerful, thick hip muscles can get overstretched from crossing our legs. The can be stressed from pressure like sleeping our our side or from car seats pressing on them. If you notice more pain when you cross your legs or ride in the car, take action to stop the pain.

The hip muscles can become tight from being overused or overstretched, too.

Our hip muscles can get out of balance sometimes. Our job is to keep them as balanced and in neutral as possible. When our muscles are in a neutral position, we have less hip pain. (And, "neutral" doesn't mean sitting in a chair all the time!)

"Neutral" means all of the muscles are balanced. They work together in harmony. Some are not stronger or weaker than others. They are all as strong as nature designed them to be so they can work together efficiently.

Some people do have a severe arthritic condition which causes hip pain and some have rheumatoid arthritis (which affects all of the joints in the body.) In those cases, a doctors' help may be necessary.

Some of us have a hip joint which has been worn over the years by the stress of having a short leg. A short leg causes pressure on the joint and muscles around the hip joint. Even this can often be "cured" by a knowledgeable muscle therapist and a lift for the short leg.

For the rest of us, here's the plan.

Think of your hip as having four sides. Front of your leg, back of your leg, the inside of your thigh and the outside of your thigh.

Your hip joint is most likely where you feel your pain. It seems like it's in the hip joint, but the cause could very well be the muscles on the inner side of your thigh or your back side.

You can start a movement plan to get your hip muscles back into balance. Always do your moves thoughtfully. Observe how your muscles are feeling. Pay attention to how you feel after your movements, too.

Move or lift your leg from each of the four sides. Do this while you are standing or on your side in bed.

You may discover, simply by paying attention to how your body feels, that it needs more movement in one direction than the others. Don't overdo, but do thoughtfully. Is one area tighter than the others?

If you determine that your hip muscles seem related to your hip pain (and they most likely are,) you can continue your movement therapy. Simply moving your legs in every direction, instead of always using them in only one or two directions, can help get your hips back to neutral.

You also might want to see a muscle or massage therapist. He or she will be able to loosen the muscles around your leg and hip bones. Sometimes those thick, powerful muscles need a little extra help to get back into balance. A massage therapist can be a perfect helper.

Relief From Joint Pain Is Not Difficult to Find


Joints in the body are prone to damage because they are constantly moving. As the human body advances with age the joints lose their lubricating fluids and make an individual prone to the problem of arthritis. Popular belief has it that people who suffer from this problem will have to lead a life of discomfort and suffer from pain. It is true that there is no medication, which can provide permanent relief from this problem. Contrary to popular belief, it is not at all difficult to find relief from the problem of arthritis. Today, there are numerous products available in the market which is proven to be effective at this issue. People only have to visit the neighborhood supermarket or grocery to make a purchase of such products. Perhaps the only caution which people will have to exercise will pertain to the type of product they choose. They must exercise a degree of caution to ensure that they do not waste money on products, which cannot deliver on the promises they make. It is essential for them to choose products that are fresh, have been manufactured in line with prescribed guidelines and are proven to be effective against arthritis. Shopping around for such products will not be a difficult affair either because people can today use the Internet for this purpose. BV Symptoms - The Tell Tale Signs and Symptoms For Bacterial Vaginosis Sufferers


Noticing that some things are off with your body, wondering what's going on, fearing the worst. Obesity Causes Pain in the Back


It is a well known fact that overweight is a major risk factor for a number of noncommunicable diseases such as cardiovascular diseases, diabetes, musculoskeletal disorders and some cancers. The risk for these noncommunicable diseases increases, with the increase in BMI (Body Mass Index). But not many people know that overweight can also cause people pain in the back.

Recently, researchers at The University of Hong Kong found that being overweight and obese significantly contributes to the development of disc degeneration. Degeneration of the joints of the spine, known as the intervertebral discs, is one of the major causes of low back pain. Traditionally, it is believed that it occurs with only aging.

Two studies for adults, one cross-section, one time series, were conducted on this issue.

The cross-sectional one was the study of adults with age 21 or above. A total of 2,599 individuals (1,040 men and 1,559 women with mean age of 42) were assessed with magnetic resonance imaging (MRI). Participants were from diverse social and economic backgrounds and were recruited regardless of whether they had lower back pain or not.

After the assessment, it was found that 73 per cent of them had disc degeneration with men (76%) having a significantly higher prevalence of degeneration than women (71%). Increasing age was also found to increase its prevalence.

Of those who had disc degeneration, 7 per cent were underweight, 48 per cent in the normal weight range, 36 per cent overweight and 9 per cent were obese.

Overweight and obese individuals were found to have a greater extent and severity of disc degeneration in comparison to those individuals of normal weight or underweight.

"Our research confirms that with elevated BMI, there is a significant increase in the extent and global severity of disc degeneration. In fact, its end-stage with narrowing of the disc space was more pronounced in obese individuals," said Dr. Samartzis, one of the team leaders of this study.

In another study, 129 working middle-aged men, representing three occupations (machine drivers, construction carpenters and office workers), were recruited. The selection was based on the age (40-45) and place of residence. MRI images of the lumbar spines were obtained two times, one at baseline and another at 4 years later (follow up stage).

In this study, besides measuring their current weight, the participants' weight at age 25, their height, history of car driving, smoking, and back injuries were also obtained.

The findings of the study showed that persistent overweight (with BMI ≥25 kg/m2 at the age of 25 and at the current age) associated strongly with risk of increased number of lumbar disc degeneration.

Overweight at young age was a stronger predictor of an increase in the number of degenerated discs during follow-up than overweight in middle age.

Thus it can be concluded that overweight with BMI above 25 kg/m2 increases the risk of lumbar disc degeneration.

This is the case for adults. As more children are now becoming overweight, how detrimental would this also affect their lumbar spine?

Another cross-sectional study was on adolescents who were 13-20 years of age. The sample size was 83.

It was found that 35 per cent of them had disc degeneration. The majority (93%) of these adolescents with disc degeneration experienced low back pain and decreased physical function.

More importantly, being overweight in adolescence triples the likelihood of having disc degeneration and developing more severe forms of the condition than those with normal weight. Individuals with juvenile disc degeneration had an increased prevalence and greater intensity of low back pain and/or sciatica and hence greater physical disability.

Also, the development of disc degeneration in young age may potentially contribute to its greater severity early on in life and possibly a life-time of low back pain episodes.

Disc degeneration is irreversible and may cause a lot of long-term suffering and psychological distress for patients. Serious low back pain may prohibit the patients to maintain normal work and social life. And so it could lead to a serious need for back surgery.

Because of more weight, physical loading on the disc and/or a chronic low-grade inflammation from the fat cells may play a role in its degeneration, Dr. Samartzis explained.

Thus there is a need for the overweight people to employ successful weight loss strategies to prevent suffering from these diseases.

Arthritis Treatment: Focus On Muscular Stiffness, Joint Swelling, And Cartilage Repair


Effective arthritis treatment reduces inflammation in arthritic joints and creates an atmosphere for the body to repair its own cartilage deterioration. The best way to reduce inflammation is to lengthen all the muscles that attach to or surround the arthritic joint. Dr. Arthur Grayzel, former medical director of the Arthritis foundation is quoted in the book Keys to Understanding Arthritis, by Elizabeth Vierck. Dr. Grayzel states that "as people age, muscles lose their elasticity, leading to a reduced range of motion (page 11)." Limited range of motion causes cartilage deterioration. As the area of joint movement decreases, the joint begins to burrow a hole in the cartilage because it constantly treads in the same small area. Active Isolated Stretching therapy increases the range of motion in arthritic joints. Dr. Sokoloff states that "new cartilage could grow on bones but the real problem is the stress on the joint that keeps intervening and preventing this process (page 22)." Conventional stretching, pnf stretching, yoga, and Thai massage have been incompetent in lengthening muscles. Optimal stretching cannot occur if the stretch hold is done for more than two seconds. Stretches done in repetition are better than one long stretch. The target muscle cannot stretch if the muscle is contracting, so stretch position must be reexamined.
